EBP Forms, Applications, and Instructions
All applications for the EBP must be submitted electronically
through the Bank's online application system by the date
and time specified for each round.
The Bank is pleased to announce the 2010 EBP funding round. A minimum of $1 million will be allocated to the program.
The online application process opened on Monday, March 29 and will close on Friday, April 16 at 5:00 p.m. Applicants will be notified of awards by Friday, April 30, 2010.

Supporting documentation must be uploaded for each application
or if accompanied by the cover sheet generated upon
completion of the online application faxed to the
Bank by the close of the round. No exhibit materials will
be accepted after the application deadline, regardless of
whether they are uploaded or faxed.

Enrollment and Disbursement Requests
Once you have been approved to participate
in the EBP, all home-buyer enrollments must
be submitted online through the Forms and Applications web
page. Select the "Equity Builder Program (members
only)" link. Once you have logged in, continue to the “Manage
My Approved Application/Enrollment” section to submit
the enrollment or disbursement request.
The following forms and instructions are required when establishing
and calculating a household's income, and when drafting the
retention mechanism.
Guidelines for Determining Household Income
These income guidelines/standards must
be used by members for the calculation of household income.
The same standards are used by the HCI staff to review and
approve household income prior to accepting the enrollment
of a home buyer.

Documentation Checklist
To assist with a member's review of an enrollment or disbursement,
this checklist is used internally by CIMs to verify agreement,
enrollment, and disbursement documentation and calculations.

Mortgage: CT, ME, VT
This Standard Retention Mortgage is for use with properties
in Connecticut, Maine, and Vermont. Please use this form
in combination with the EBP Standard Retention Note. In
Maine and Vermont, the mortgage must be recorded; in Connecticut,
both the mortgage and the note must be recorded.

This Standard Retention Mortgage is for use with properties
in Massachusetts, New Hampshire, and Rhode Island. Please
use this form in combination with the EBP Standard Retention
Note. The mortgage must be recorded in all three states.

All six New England states must use this note. In Connecticut,
this note must be recorded.

Recapture Payoff Worksheet
Members can use this calculation worksheet to estimate
the payoff requirement in the event of a sale of property
supported by EBP funds prior to the end of five years.
Members are required to contact their community
investment manager prior to sale to verify the
actual payoff requirement.
